Lack of Clinical Utility of the Clinical Stroke Scores

Abstract
Purpose: A correct diagnosis of stroke is important for immediate treatment. The aim of this study is to determine the accuracy of two clinical stroke scores (Siriraj Stroke Score and Guy¡¯s Hospital Score) in patients with acute stroke.

Method: Fifty two patients with acute stroke confirmed by D-WMRI were analyzed by Siriraj Stroke Score (SSS) and Guy¡¯s hospital score (GHS). We evaluated the sensitivity and specificity between the two clinical stroke scores.

Result: Forty six patients (88%) had infarctions and six patients (6%) had hemorrhages. The sensitivity of SSS was 89% for infarction (equivocal cases were excluded). The GHS had a sensitivity of 88% for infarction. Both SSS and GHS could not detect hemorrhage.

Conclusion: In this study, neither of the two clinical stroke scores is sufficiently accurate for the differentiation of stroke type. We suggest that scoring systems that only require a small number of variables are unlikely to achieve the level of accuracy needed for clinical decision-making.
